Treatment History Forms

Obtaining an accurate and reliable history of all previous treatment(s) is a crucial part of how we work. It not only summarises the individual's past treatment, but also informs decisions about future treatment.

To achieve this, we use a range of tools that we make available here. They can be updated as more treatments are tried, and it is possible to record important features of the person's contact with services, along with information on illness episodes and admissions to hospital.

Antidepressant Treatment History Form (ATHF)

The ATHF consists of scoring instructions and ratings for most antidepressants/ augmentation/ and ECT trials. It is being used increasingly to determine the adequacy of antidepressant trials.

It has been updated as the ATHF Short Form (ATHF-SF), so the version here is commonly referred to as the ATHF Long Form.

OCD Treatment History Form (OTHF)

The OTHF was developed by us as a practical tool for assessing the adequacy of previous treatment for OCD.

The OTHF is a theoretical advance on the Antidepressant Treatment History Form (ATHF) and follows similar principles. Those acquainted with the ATHF will find scoring of the OTHF to be familiar. The main difference between the two tools is that when using the OTHF, adequacy rating is based on dose and duration whilst the ATHF prioritises dose as long as the trial is longer than 4 weeks. When rating the adequacy of treatment for OCD, both dose and duration are important.
