  • Send a cover sheet along with the fax, containing a confidentiality statement.
  • Only the minimum amount of personal information necessary should be sent by fax.
  • Where possible, information should be anonymised and a unique identifier used.

If you are a clinician, sending patient-identifiable information via this form is likely to represent a breach of patient confidentiality and data protection laws. Remember that date of birth, CHI number, postcode etc is patient-identifiable information. The CHI not only contains the date of birth but also the sex of the patient. Instead, send us a general enquiry and we will reply to you from an NHS email address that allows patient-identifiable information to be transferred between NHS email addresses.

If you do not work in the NHS, please note that we may not always be able to answer queries about psychiatric treatment in general. Please read other sections of the website to ensure that your query is appropriate for the service. In many cases, it may be more appropriate for you to ask your GP, Community Psychiatric Nurse (CPN), support worker, or psychiatrist about the treatments available to you.
