Use of online forms

These are provided as an easier way for patients already being seen or treated by the service. They should not be used for diagnostic purposes and they do not replace competent clinical assessment. No patient identifiable information should be included and anyone invited to complete them will have been given a unique ID.

Use of rating scales

If you use any rating scales, please be aware that decisions about diagnosis, severity of symptoms, and treatment remains with the person overseeing the patient's care. We cannot be responsible for any decisions being made by third parties and without express advice from the Advanced Interventions Service.

Guidelines and algorithms

These are intended to guide and inform clinical decisions about treatment. It is rarely advisable to blindly follow an algorithm and all treatment decisions have to be made according to relevant information (e.g. previous treatments, tolerability) that cannot always be incorporated into treatment pathways. The information provided here is not a replacement for clinical guidelines produced by guideline panels. Responsibility for treatment decisions always lies with the treating clinician.

Proformas and checklists

We provide these to assist clinicians in providing us with the key information we might need in order to consider a referral to the service. If you are completing these, please do not send them via insecure/ unencrypted email. Always ensure that information sent to us is via a secure route (such as NHS mail). If unsure, post it.
